Social determinants of health are social circumstances that impact our health. These include among others: food insecurity, lack of transportation, educational challenges, lack of cell or house phone, lack of insurance , domestic violence, lack of child care, lack of recreational space and activities, unstable housing, no employment. These factors affect a patient's ability to obtain medical care and follow through with prescribed heath plan to keep well. It is a well recognized fact that simply giving a diagnosis and prescription to a patient will make them well.
The Aloha Health Model addresses these and other social determinants and ensures that residents are brought into care and ensures maximum compliance of patients through community leaders before a health crisis occurs.
